All patient forms are completed online before your appointment. You can also upload your medical aid card and ID at the same time.
A questionnaire covering your previous hearing history should also be completed — including any additional information you feel is relevant and a list of all medication you currently take.
Bring any previous hearing test results or existing hearing aids with you when you come in.
If you're coming for a dizziness assessment, specific pre-test instructions will be sent to you — certain food, medication and drinks must be avoided for the 48 hours prior to your appointment, so they don't mask the results.
